Personal Care Attendant

Case Coordinator/Office Staff
This position involves managing and overseeing individual client cases from the initial assessment through ongoing care. The Case Coordinator works closely with our people served, families, and caregivers/Personal care attendants to develop personalized care plans, coordinate services, and monitor progress. They serve as the primary point of contact, addressing any concerns and making adjustments to care as needed. Strong organizational, communication, and problem-solving skills are essential, as the Case Coordinator ensures that our people served receive consistent, compassionate care that enhances their well-being and quality of life.
